Some important info before arriving at your appointment:

• Patient must be present for appointment. Parent cannot bring form to appointment without child present regardless of how recently a child has been evaluated by their PCM.
• Do not bring additional children to the appointment. If additional child(ren) require a sports physical, each child requires their own appointment.
• Parents must bring physical forms with them to the appointment. Parents must fill out their portion of the paperwork prior to arrival. If incomplete, this may result in loss of scheduled appointment time. Lyster does not have copies of local school forms. Be sure to get the form from the school your child is attending. Different schools have different forms.
• Only school/sports physicals will be addressed. If there are other concerns or issues (medication refills, referrals, acute issues, etc), please make an appointment during regular clinic hours.
• If the child wears glasses, please bring glasses to appointment for eye exam portion of physical.
